Prescriptions
On a 6 monthly basis we need to update your list of prescribed medication on your medical records.
All prescription requests are reviewed and responded to by a doctor. If you have attended the clinic or hospital out patients in the previous 6 months, and the doctor is happy that you have been properly followed up, you may be issued a repeat prescription. If you are due a review you may receive a text, or note attached to a single prescription, asking you to book a routine appointment with a Doctor before your next prescription is due.

To request a repeat prescription please download the repeat prescription request form below and email it, along with the name of your pharmacy to:
Your prescription will be forwarded to your pharmacy of choice 48 hours after your request. It is advised that you contact your pharmacy before you arrive to collection your medications
